Method 1 of 2: Making the Model
Step 1. Draw the pattern
The outline of a template is quite simple and you can trace it freehand; alternatively, use a template that you can download from the internet.

Cut out the outlines.
Step 2. Place the pattern on the fleece or cotton fabric
Cut out two identical pieces of fabric using the template as a reference.
Method 2 of 2: Assemble the Faceplate
Step 1. Use fabric glue to join the two pieces of fabric (alternatively you can use pins)
Step 2. Sew a seam to hem all the way around the edges of the two shapes to complete the template
Step 3. Join the elastic by sewing both ends on each side of the fabric (this way you can wear the mask)
Step 4. Finished
Making this simple template shouldn't take more than a few minutes to cut and sew; put it on and take a nap.
