A personal data sheet provides biographical and logistical information about a person, including contact information as well as details relating to past residences, educational background and social activities performed. The purpose of a personal data sheet is to provide a brief snapshot of your person and origins. It can be used to accompany job applications or school forms. Major organizations often have their own forms relating to personal information, but sometimes it will be possible to provide your own data sheet. Method 1 of 1: Write Your Personal Data Sheet
Step 1. Start with your name and contact information
Each personal data sheet should contain your full name at the top of the first page. If there are additional pages, enter your surname on each of them. Add your current address, as well as your permanent address in case they don't match. Include any phone numbers of yours, such as your home, office, or cell phone number. Also provide an email address.
Step 2. Provide information about an emergency contact
Enter the name, address and telephone number of at least one person to whom you can contact in an emergency. It is important that this information is accurate and updated constantly. These details must be usable even if you are not reachable for any reason.
Step 3. Confirm your identity with a driver's license or social security number
People are often reluctant to provide sensitive and private information such as a social security number. However, the purpose of a personal data sheet is to enclose all your information in a single document. Include one or both numbers to confirm your identity.
Step 4. List your educational background and experiences
Include the schools you attended and your current and past work experience. If you are still a student, or if you have just finished your studies, start by listing your educational background. If your stay in the world of work is already quite substantial, highlight your career by entering it as first information.
Step 5. Add those special skills that can identify you
Do not forget the foreign languages known, the licenses or certificates obtained, as well as any prizes won. You can list enrollments, publications, or particular social or volunteer activities.
Step 6. List the references
Include at least 3 references related to your work and school career. Provide their name, contact, title and duration of your partnership.
Advice
- Do not forget to ask for the authorization of the people you are going to indicate as references. Don't let them be surprised when an information call arrives.
- Include any additional information that may be needed based on your use of the card. For example, if you need to bring it to the attention of a sports organization, you could include physical information about your weight and height. The military may want to know the color of your eyes and your closest relatives. Customize the card to provide all the necessary information you want to share.
- Structure the personal data sheet to make it easy to read and fill out. You could use a text program that can make your document look like a resume. Alternatively, if you prefer the world of numbers and fields, choose a spreadsheet.
