Getting a copy of a medical record after hospitalization can be difficult if you don't know how to make this request.
Steps
Step 1. Decide which documents you need, or which hospitalization you need a copy of the medical record
Step 2. Fill out the application form in your name, or by designating a delegate for the withdrawal of the file
Step 3. Specify dates and department of competence, inserting your data complete with address, social security number, and anything else that may be useful to uniquely identify the hospitalization
Step 4. Go to the appropriate office in person, usually at the hospital where you were admitted, and find out about the costs and times to have copies available
You will not be allowed to copy documents in person from the health facility archive.
Step 5. Obtain a receipt indicating the date you will be able to collect the copy documents
Step 6. When you go to collect the copies, make sure they include all documents and examinations relating to the hospitalization

- In the event of multiple hospitalizations, specify the dates of the event for which copies of the medical record are required, so as not to create possible inconvenience or errors on the part of the office in charge.
- Be aware that copies of specific medical printouts, such as plates or MRI scans, may be expensive.
- Specify what you need, detailing whether to include costs incurred, handwritten notes from doctors, or other relevant information.
