Steps
Step 1. Prepare your National Statistics Office (NSO) Certified Copy (SECPA, Security Paper) Birth Certificate (BC)
Call the NSO hotline at (02) 737-1111 for your birth certificate.
Step 2. Prepare three (3) valid identification documents:
driver's license, SSS ID (Social Security System ID), BIR (Bureau of Internal Revenue) ID, school card and other supporting documents that include your first and last name, your date of birth and your home address.
Step 3. Go online and make an appointment with the DFA
Fill out the application form, download and print it when you're done and always remember your appointment reference numbers.
Step 4. Make two (2) photocopies of each document in your possession (NSO birth certificate, valid ID, application form, and other supporting documents)
Step 5. Make sure you arrive on time for your DFA appointment
Step 6. Present your appointment form at the appointment desk
Step 7. Get your queue number at the information desk and sit in the designated seat while you wait for your turn
Step 8. After data processing, you can proceed to the passport registration section located on the 2nd floor and pay the passport issuing fee
(Regular Issue Fee: Php 950, 00 (25 working days): Rush Issue: Php 1,200, 00 (15 days) An additional fee of Php 200 will be charged for the lost passport issue, if it is still valid.
Step 9. Next, proceed to the coding section for data capture
Step 10. Wait for the passport to be delivered directly to your home (optional)

Advice
- In the meantime, for the loss of the passport, it is necessary to execute a loss affidavit. Also prepare photocopies of your lost passport and bring a copy of your NSO birth certificate and other supporting documents with you.
- For the renewal of the passport, it will be sufficient to photocopy the front and the last page of the passport (two copies per page). Also prepare a copy of the NSO birth certificate.
